This Case Manager position is responsible for facilitating care delivery and timely discharge or transfer of patients. Key responsibilities include:

• 
  
• Managing patient flow to ensure care is appropriate, authorized, and conducted in the most suitable setting
  
• Providing clinical information and insight to healthcare team members and payer personnel
  
• Serving as an internal resource to Revenue Cycle on clinical issues and liaising with external case managers, review organizations, and insurance companies
  
  
  This role requires flexibility in hours and a strong ability to manage activities and multiple priorities within complex systems. The ideal candidate will possess:

  • Exemplary communication and problem-solving skills
    
  • Strong computer skills and typing ability (minimum 40wpm)
    
  • Ability to manage ambiguity, uncertainty, and change
    
  • Tolerance for unstructured environments
    
    
    Requirements include:
    
    
    • 
      
    • Bachelor of Science in Nursing (B.S.N.) from an accredited college or university (degree in progress may be considered)
      
    • Minimum three years of recent relevant clinical experience, with experience in case management and strong discharge planning skills
      
    • Current unrestricted Registered Nurse (RN) license issued by the California Board of Registered Nursing (BRN)
      
      
      This is a Per Diem, Part-time position working Days.
