Job Description:
A company is looking for a Care Manager I, Specialty
Programs.
Key Responsibilities:
Assess member's clinical needs against established guidelines for
specialty populations
Implement high-quality, cost-effective care based on customized
population models and clinical practice guidelines
Coordinate with members, families, and healthcare teams to ensure
comprehensive care and resource utilization
Required Qualifications:
License as an RN or a Bachelor's degree in social work, health, or
behavioral science
Graduate level degree in Social Work or Health and Behavioral
Science preferred
Minimum of three (3) years' experience serving members with special
needs
Strong knowledge of case management standards and managed care
principles
Knowledge of specialty populations as applicable
