California Healthcare Job Details Location: San Fernando, California – Access to Los Angeles, scenic parks, and a diverse healthcare community Job Type: Locum tenens (contract position) Schedule: Full-time, Monday through Friday Shifts per Week: 5 Shift Duration: 8-hour days Setting: Hybrid – 3-4 days in-home patient visits, 1-2 days remote (telehealth, admin, charting) Contract Duration: 26 weeks Hours per Week: 40 Estimated Weekly Salary: $3840 - $4550 per week Travel: Occasional travel within San Fernando and Santa Clarita; overnight stays possible (travel expenses covered) Start Date: December 8, 2025 Nurse Practitioner Qualifications Minimum 2 years of experience as an Advanced Practice Provider (Nurse Practitioner) Experience in home health, primary care, or internal medicine preferred Experience managing care plans is a plus Current and valid Nurse Practitioner license in California Ability and willingness to travel within the assigned territory Backgrounds in urgent care, hospitalist, surgical, or pediatrics are not a fit for this healthcare position Healthcare Responsibilities Conduct initial assessments, follow-ups, and hospital transitions for patients with chronic and complex conditions Perform comprehensive hands-on and head-to-toe assessments, including full vitals Provide disease education and medication management for chronic kidney and renal disease patients Visit 4-5 patients daily (occasionally up to 8); new patient visits average 90 minutes, re-visits 30-60 minutes Participate in POD rounding to discuss high-risk patients and care updates with the healthcare support team Complete documentation using Athena and Salesforce EMR platforms Utilize AI-powered scheduling tools to optimize patient visits San Fernando, CA, offers suburban comfort, urban convenience, and proximity to the greater Los Angeles healthcare market.
