Job Description:
Job Title: Case Manager - RN Profession: Registered Nurse
Specialty: Case Management Duration: 13 weeks Shift: 8x5 Days Hours
per Shift: 40 hours/week, M-F with two weekend day shifts
Experience: Preferred experience in Acute Rehabilitation Unit
(ARU). Will consider skilled nursing facility (SNF) and acute
inpatient experience. License: Active RN license in the state.
Certifications: None specified. Must-Have: Minimum years of
experience not specified. Description: The Case Manager is
responsible for coordinating patient care and facilitating
rehabilitation services. The role involves working collaboratively
with healthcare teams to develop and implement discharge plans. The
Case Manager will monitor patient progress and adjust care plans as
necessary. Strong communication skills are essential for patient
and family education. Knowledge of electronic medical records (EMR)
systems is required. The position does not require floating to
other units. Scrubs will be provided by the facility. The
assignment is a replacement for permanent staff.
