Patient ratio of 1:20. 40 hours of orientation provided. No floating required. Highlights: Work within an acute care facility focused on care coordination and discharge planning Collaborate with interdisciplinary teams including Social Services and Transportation Utilize Epic EMR for computerized documentation Serve diverse patient populations including adolescents, adults, and geriatric patients Professional dress code: scrubs or business attire accepted Up to 7 days of time off available during assignment Requirements: Active California RN license (pending licenses accepted if active by start date) Minimum 2 years of acute care hospital Case Manager experience required Demonstrated experience performing care coordination and discharge planning in a hospital setting Proficiency with Epic charting system required Experience with InterQual criteria, DRG, CMS regulations, and plan of care development Competency in arranging DME, skilled nursing facilities, and ambulance transport independently Knowledge of HMOs, IPAs, and various insurance models Proficient in Microsoft applications Experience with complex discharge planning including patients experiencing homelessness, mental health concerns, substance abuse treatment coordination, advance directives, and POLST Ability to organize and participate in family meetings Travel healthcare experience preferred; first-time travelers will be considered with clinical screening Why It's a Great Opportunity: This position offers an excellent opportunity to make a meaningful impact on patient outcomes through comprehensive care coordination in an acute care environment.
