Job Description & Requirements - Specialty: Case Management - Discipline: RN - Start Date: 02/02/2026 - Duration: 13 weeks - 40 hours per week - Shift: 8 hours - Employment Type: Travel Outpatient Oncology Care Manager One 2x8hrs every other week and 3x8hrs every other week Required: 2 years of Case Manager experience and experience with Oncology and PEDS Patient Diagnoses: There is no call requirement. Also, this job does not administer Chemotherapy, but must have knowledge of Chemotherapy, it's interactions and experience with Roadmaps. CPHON certified if possible. Float Required: Yes; covering for other colleagues Certifications: Required - PALS or PEARS Preferred - CPHON Avg. Daily Census: 20 - 22 Patient Age Group: Neonates Infants Pediatrics Adolescents Special Procedures/Unit Details: Patient Care manager role supporting the Neuro Onlcology patients. Will be responsible for working in a multidisciplinary team to develop a road map for this patient population. Accountable for assessment of patients, implementation of physician orders, treatment plans, medical procedures, record keeping, as well as monitoring and evaluating patient care processes and outcomes. Coordinate activities with other services and disciplines. Must have an Oncology background Special Equipment: Must have the following skills: Responsible for road map - will be customized per patient coordinating follow up appointments ordering and follow up of labs/appointments/scans Referrals authorization DME ordering/coordination Care Coordination Transition planning Needs Assessment Experience with Neuro Oncology patients HonorVet Technologies Job ID 31708757.
