Job Description Job Description The RN Supervisor serves as the
clinical leader for our Home Health agency, overseeing patient care
operations, staff supervision, and clinical compliance. This role
ensures all care delivered meets the highest standards of quality,
compassion, and regulatory integrity. The RN Supervisor plays a key
role in coordinating new patient admissions, quality assurance, and
communication between patients, clinicians, and administrative
staff. The position includes occasional travel to patients’ homes
to conduct supervisory, start-of-care, or high-acuity visits as
needed. Primary Responsibilities Clinical Oversight & Documentation
Review Review and finalize OASIS assessments for accuracy and
compliance prior to locking. Review, approve, and sign Plans of
Care (POC) ensuring physician orders and clinical documentation
align with regulatory standards. Conduct chart reviews and audits
to ensure completeness, timeliness, and adherence to agency
policies and Medicare guidelines. Identify documentation trends and
provide feedback or corrective action to improve quality and
compliance. Collaborate with leadership on QA initiatives,
performance improvement plans, and internal audits. Intake &
Referral Management Participate in the clinical assessment of new
referrals and intakes, determining eligibility, service needs, and
visit frequencies. Collaborate with intake and coordination staff
to ensure smooth patient onboarding and timely start of care. Serve
as a clinical resource for physicians, referral partners, and case
managers during the admission process. Field Supervision & Patient
Care Conduct Start of Care (SOC), Resumption of Care (ROC), and
supervisory visits as needed. Support field clinicians on complex
or high-acuity cases, providing hands-on clinical oversight and
guidance. Communicate regularly with patients, families, and staff
to ensure satisfaction, continuity of care, and proactive issue
resolution. Lead or participate in case conferences to align
interdisciplinary care plans. Quality Assurance & Compliance
Oversee adherence to Medicare Conditions of Participation (CoPs),
state regulations, and agency policies. Ensure consistent
compliance with documentation standards, infection control, and
patient safety protocols. Partner with the compliance and QA
departments to develop action plans addressing deficiencies or
improvement opportunities. Qualifications Current Registered Nurse
(RN) license in the State of California. Minimum 1-2 years of
clinical experience in any setting Proven ability to lead and
mentor field clinicians while maintaining quality and compliance.
Exceptional communication and interpersonal skills with patients,
staff, and physicians. Strong analytical and organizational skills
with a proactive, solution-driven approach. Valid driver’s license,
reliable transportation, and proof of insurance.
