Job Summary:Reporting directly to the Assistant Area
Administrator under the supervision of the Quality Coordinator, the
Quality Program Analyst-Care at Home is responsible for maintaining
continuous quality and performance improvement initiatives. The
role involves participating in risk management, identifying
patterns and trends in patient care issues, ensuring compliance
with Quality Improvement standards, and contributing to the
development of solutions for quality management
challenges.Additionally, the position involves integrating various
data reporting systems to create statistical reports that highlight
practice pattern variations for clinicians, leaders, and relevant
groups.
Essential Responsibilities:
- Participating in Risk Management and identifying patterns and
trends in patient care issues
- Assisting in developing Policies and Procedures to mitigate
organizational risks
- Monitoring compliance with Quality Improvement
requirements
- Assisting in designing and implementing solutions for quality
management matters
- Maintaining ongoing quality and performance improvement
projects in collaboration with the Quality Coordinator
- Coordinating and managing data collection processes
- Participating in the development of quality control and
improvement indicators
- Ensuring team members meet project deadlines
- Preparing and presenting data for multidisciplinary reviews and
team meetings
- Working closely with leadership to analyze data for performance
improvements
- Conducting quality reviews utilizing approved indicators
- Developing meaningful quality improvement indicators
- Ensuring adherence to federal, state, and local standardsBasic
Qualifications:Minimum two (2) years of experience in patient care,
medical support, clinical support, or quality improvement function.
High School Diploma or equivalent required.License, Certification,
Registration:No specific requirements. Must possess strong
consultative, communication, and management skills, critical
thinking abilities, and knowledge of healthcare standards.The ideal
candidate should have a good understanding of TJC, Title 22, NCQA
and CMA standards, as well as TQM principles, medical care
practices, medical records, statistical analysis, and data
management. Proficiency in relevant software is essential.
Preferred Qualifications:
- Preferred certifications or licenses in relevant fields
- Experience in Home Care, Palliative Care, or Hospice
- Knowledge of performance improvement methodologies
- Experience with Electronic Medical Records
- Experience in a large healthcare system
- Medical Terminology certification
- Degree in data analytics or health-related field
