Care Program Job Overview Overview We are seeking a compassionate
and mission-driven Licensed Clinical Social Worker (LCSW) or
Licensed Marriage & Family Therapist (LMFT) to join our PACE
(Program of All-Inclusive Care for the Elderly) team in San Marcos,
California. This full-time, onsite role focuses on delivering
individualized behavioral health services through a mix of
center-based counseling and participant home visits. The Behavioral
Health Specialist plays a key role in assessing mental health and
cognitive functioning, developing treatment plans, and supporting
participants’ emotional and psychosocial well-being. Working as
part of an interdisciplinary team, this clinician provides both
individual and group counseling while promoting independence,
dignity, and quality of life for older adults. Responsibilities
Perform comprehensive psychosocial and behavioral assessments for
PACE participants Develop and implement individualized treatment
plans based on participants’ mental health and functional needs
Provide one-on-one and group counseling using evidence-based
interventions Monitor participant progress, modify treatment plans,
and coordinate ongoing care Collaborate with the interdisciplinary
team (IDT) to ensure integrated care planning and follow-up Educate
participants and staff on coping strategies, stress management, and
trauma-informed care Document all assessments, interventions, and
care plans accurately and in a timely manner Represent the PACE
program at community events, outreach efforts, and professional
presentations Participate in quality improvement and staff
