Job Description:
A company is looking for an Oncology RN Case Manager -
Remote.
Key Responsibilities
Make outbound calls to assess patients' current health status
Identify gaps or barriers in treatment plans and coordinate care
for members
Provide patient education and document findings related to disease
processes
Required Qualifications
Current, unrestricted RN license with the ability to obtain
multiple state licenses
3+ years of recent Oncology experience in a hospital or acute care
setting
Recent chemotherapy administration experience
Computer proficiency, including data entry skills and navigating a
Windows environment
Access to high-speed internet and a dedicated workspace for home
office setup
